Medical Assistant is to assist primary care providers by delivering

Profile Overview Under the guidance of the primary care operations
team, the Primary Care Patient Care LPN collaborates closely with
our Primary Care Providers to ensure safe, timely, effective,
efficient, and equitable patient-centered care is delivered
systematically. The LPN will work alongside patients, providers,
and other clinical team members to enhance quality outcomes for all
individuals. Responsibilities will be carried out in alignment with
our mission, values, and service standards. Position Details:
Minimum Qualifications: Education: Completion of an accredited
practical nursing program. Licensure: Must hold a valid LPN
(Licensed Practical Nurse) license in the state of practice.
Preferred Experience: At least 6 months of clinical healthcare
experience, ideally in a primary care setting. Additional
Knowledge, Skills, and Abilities: Ability to work effectively as
part of a team and collaborate with all care team members. Strong
written and verbal communication skills. Customer service-oriented
mindset. Excellent organizational and interpersonal abilities.
Positive and flexible attitude. Attention to detail and accuracy.
Self-motivated and capable of performing tasks independently within
the scope of practice with minimal supervision. Physical
Requirements: Regularly required to push, pull, and/or lift up to
50 pounds. Must be able to stand and walk for extended periods
during each shift. Ability to grip, reach, bend, kneel, twist, and
