Physician Position: The Primary Care Physician delivers
value-driven healthcare primarily to Medicare beneficiaries at
community-based primary care facilities, emphasizing high-quality

teams, and maintaining electronic health records. Candidates must
possess a valid medical license, board certification or eligibility
in Internal or Family Medicine, along with experience or a keen

Organization: Our Company Role Overview: The role of a Primary Care
Physician within our organization is to deliver equitable and
effective value-based healthcare to local Medicare populations at
our innovative network of community primary care centers. Our
physicians work within a single medical facility, supported by
extensive care teams, allowing them to concentrate on enhancing the
quality of care rather than merely increasing the volume of
services provided. Our value-based care framework and competitive

candidates may also be considered for a Center Medical Director
role. Key Responsibilities: Evaluate and diagnose patients at our
local clinics. Oversee and manage primary care services. Prescribe
and administer medications and treatments. Maintain patient
electronic medical records using Canopy and Greenway. Collaborate
with regional and central leadership to achieve health quality
objectives. Work alongside Practice Managers to lead and manage the
center's care team. Perform additional duties as assigned.
Essential Qualifications: Graduation from an accredited Medical
Doctor (M.D.) or Doctor of Osteopathy (D.O.) program. Board
Certification in Internal Medicine or Family Medicine (or board
eligible). Possession of an active, unrestricted state medical
license. Active DEA registration. Authorization to work in the U.S.
Preferred Qualifications: Completion of a fellowship in Geriatrics.
Experience in treating geriatric populations. Familiarity with
value-based healthcare models. Experience in collaborative
environments to ensure positive health outcomes. Background in
outpatient primary care settings. Bilingual skills are a plus where
applicable. Expected Weekly Hours: 40 Employment Type: Full-time
Salary Range: The anticipated salary range for this position is:
$174,070.00 - $374,920.00 This range reflects the base hourly rate
or annual salary for all roles within the job grade applicable to
this position. The actual salary offered will be influenced by
various factors, including experience, education, location, and
