JOB DETAILS Valley View Health and Rehabilitation specializes in providing a "home-like environment" with professional nursing and therapy services for both long-term care and rehabilitation. This means you'd be working with residents over an extended period, allowing for the development of deeper relationships and a more comprehensive understanding of their needs. We are seeking a skilled and compassionate Admissions Nurse to join our team. The Admissions Nurse will play a key role in welcoming new residents, coordinating admissions, and ensuring a smooth transition into our facility. This position requires excellent clinical skills, attention to detail, and a commitment to resident-centered care. Responsibilities Conduct initial assessments of new residents to determine care needs and develop individualized care plans. Coordinate with families, physicians, and other healthcare providers to gather necessary medical information and ensure seamless admissions. Educate residents and families about the facility’s services, policies, and care processes. Document resident information accurately in compliance with state and federal regulations. Collaborate with the interdisciplinary team to ensure continuity of care. Provide emotional support to residents and families during the transition process. Participate in quality improvement initiatives to enhance the admissions process. Qualifications Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in [State]. Minimum of [1-2] years of nursing experience, preferably in long-term care or geriatrics. Strong assessment and documentation skills. Excellent communication and interpersonal skills.
