Job Description:
Encompass Health Corporation, a leader in integrated healthcare
services, is seeking a dedicated Case Manager II for our
comprehensive network of inpatient rehabilitation hospitals and
home health services. This role focuses on facilitating effective
patient care planning across various settings, ensuring the
delivery of high-quality treatment and cost-effective outcomes. The
Case Manager II will liaise with healthcare professionals and
support teams to coordinate all aspects of patient care from
admission to discharge, including the transition to home care as
needed. Develop and review treatment plans in collaboration with
medical and healthcare teams. Regularly evaluate patient progress
and adjust care plans as necessary. Provide comprehensive patient
education regarding treatment processes and home health care.
Effectively communicate with healthcare providers, patients, and
their families to ensure clear understanding of treatment plans.
Facilitate care transitions between healthcare facilities and home
services when appropriate. Document all patient interactions,
treatments, and outcomes accurately. Advocate for the needs and
best interests of patients. Strong skills in patient assessment and
care planning Excellent communication and interpersonal skills
Ability to manage multiple cases with attention to detail
Capability to work collaboratively in a team environment
Proficiency in utilizing healthcare management software Medical
Insurance Dental Insurance Vision Insurance 401k Certified Case
Manager (CCM) preferred Licensed Registered Nurse (RN) or licensed
in a clinical healthcare discipline in the state of employment
First Aid and CPR certifications
