Job Description Primary Care Physician Compensation: Base Salary
$215K–$245K Quality Incentive Bonus We’re seeking a dedicated
Primary Care Physician to join our growing network of neighborhood
clinics delivering high-quality, value-based care to Medicare
patients. Our model emphasizes outcomes over volume—empowering
physicians to focus on preventive medicine, patient relationships,
and long-term wellness. Supported by robust care teams and advanced
technology, our providers can deliver the kind of care that makes a
real difference. Position Highlights Care Team: Scribe, Medical
Assistant, and Social Worker support Onsite Resources: Lab with
phlebotomist; RubiconMD specialist consult access EMR Systems:
Canopy and Greenway Model: Value-Based Health Care Clinic Team: 3–5
providers per site (mix of MDs and NPs) Community Engagement:
On-site wellness programs—exercise, yoga, dance, and social events
Patient Services: Complimentary transportation within 5 miles of
the clinic Compensation & Benefits Physician Partnership Track 6
