Job Description Job Description Salary: JOB SPECIFICATIONS:
Graduate of an accredited School of Nursing with a current
unencumbered Nursing License in Alabama as a Licensed Practical
Nurse. License maintained with timely completion of CEUs and
licensure acquisition. A minimum of one (1) year of experience as a
LPN in a healthcare facility is preferable. A valid and current
Alabama Driver's License without felony driving convictions and
access to a reliable automobile. Current CPR card. Demonstrated
ability to plan, organize, and implement patient care as Team
Leader in coordination with the members and following physician
orders. A demonstrated understanding of diagnosis related clinical
care provision of services with the ability to utilize this
knowledge to provide quality and safe delivery of Home Health care
services. The demonstrated ability to promote and provide Home
Health care services that reflect the Tombigbee Healthcare
Authority mission and Home Health Agency philosophy and standards,
and nursing care of all patients. POSITION RESPONSIBILITIES:
Assess, distinguish, report and act upon the physical, social,
developmental and environmental needs of all patients; and the
knowledge to provide age specific healthcare to patients as
appropriate and necessary. Initiates and develops the patient plan
of care with documentation that reflects the delivery of quality
and safe Home Health Services which includes Case Conference input
and accurate, timely and concise documentation. Coordinates all
services including the recognition and referral to all services
needed (in coordination with the physician) to provide and promote
total patient well-being. Exercises proper infection control and
safety policy and procedures including proper bag technique and
appropriate hand hygiene during all phases of patient care.
Educates patients and significant others regarding the patient's
illness, procedures, treatments, medications, including self-care,
prevention, and health maintenance with accurate, concise and
timely documentation. Client concerns are routinely and
consistently addressed with demonstrated patient confidentiality.
Follow-up is done timely and as needed with the appropriate
healthcare provider. Assignments are organized with all paperwork
turned in as required prior to initiating daily Home Visits and
timely submission of paperwork upon return. Weekly assignments are
managed without incident during on-call weeks. Makes sound nursing
judgments based on observation and factual information and seeks
guidance and direction as necessary for performance of duties,
recognizing priorities in patient care. Recognizes and performs
duties which need to be performed but not directly assigned.
Maintains all equipment in good repair and proper utilization of
equipment and supplies in a cost-effective manner is demonstrated.
Provides timely notification of malfunctioning equipment or company
automobiles. Exercises sound fiscal expenditure judgment through
the proper management of patient and department supplies and
revenue judgment through the management of visits to prevent missed
visits when possible. Promotes and maintains an agency environment
that is in compliance with federal, state, and local rules and
regulatory agencies. Actively assists with all required surveys.
Assists in coordination and implementation/maintenance of
activities that contribute to the attainment of the organization
wide quality assurance and quality improvement objectives, taking
into consideration the physical, social development, and
environmental needs of all patients admitted to Home Health.
Demonstrates understanding of emergency procedures and
internal/external disaster policies and procedures. Presents a
well-groomed, professional appearance, observing dress code.
Maintains environment (company car when driven and work desk) in
order. Adapts to unavoidable changes in workload, schedule, or
assignment in a professional manner. Accepts constructive criticism
and suggestions in a positive manner. Reliable in reporting to
work, keeps absenteeism to a minimum. Provides notification for
absence and tardiness in a timely manner. Willingly accepts
additional work assignments as requested. Willing performs other
duties as delegated to exercise the mission of the Authority
PHYSICAL REQUIREMENTS: Good verbal and written communication
skills. Visual/hearing ability sufficient to comprehend
written/verbal communication. Ability to perform tasks including
physical activity which may include heavy lifting, i.e. patient
from bed to wheelchair or to bedside commode, and extensive bending
and standing, i.e. range of motion, dressing changes.
